Yale-New Haven Hospital news release
Yale-New Haven becomes one of first U.S. hospitals to use new drug treatment for removal of brain tumors

For more information, call 203-688-2493 or E-mail Mark D'Antonio

Yale-New Haven Hospital is currently one of only four hospitals in the United States using the drug IL 13-PE38QQR (IL 13-PE toxin) in clinical trials for the treatment of malignant glicoma (primary brain tumors).

The drug IL 13-PE toxin is being tested in the treatment of patients with a malignant brain tumor that has recurred and for which another operation is planned.

IL 13-PE toxin is a "cytokine," a chemical normally produced by the body. Certain cancer cells have a special place on their surface, to which IL 13 can attach. Pseudomonas extoxin (PE) is a protein made by bacteria which kills tumor cells if it gets inside. IL 13-PE toxin is made up of IL 13 and PE. Doctors at Yale-New Haven are hopeful that the IL 13 will attach to glicoma cells, allowing the PE to penetrate and kill the cancer cells.

"We are hoping this new treatment for brain tumors will accomplish several important things," said Dr. Joseph Piepmeier, director of neuro-oncology and vice chairman of the department of neurosurgery at Yale-New Haven Hospital. "As part of this clinical trial, we hope to determine the dose of IL 13-PE toxin effective for killing tumor cells and the effects related to that dose when given before surgery. We also need to determine any side effects of IL 13 when given at doses up to the effective dose and given for a longer period of time into the surrounding tissue after surgical removal of the tumor. Finally, we want to see the effectiveness of IL 13 in preventing tumor recurrence."

The surgical procedure begins with an infusion of IL 13 into the tumor one week prior to the planned removal of the tumor. The surgical team uses an MRI scan to plan the exact location of the catheter tip, then inserts a small tube into the brain through a small hole in the skull. The IL 13-PE toxin is administered over the next two days using a small pump and surgery is scheduled for one week later. Analysis of the tumor tissue may show that IL 13-PE toxin can kill the tumor cells. In successive groups of patients, the amount of IL 13-PE toxin will be increased in steps. This increase in drug dose is stopped when an effective concentration is determined, or if serious side effects occur. The spread of tumor cells into normal brain matter is the biggest obstacle to treatment. Therefore, additional IL 13-PE toxin is administered after surgery into the brain adjacent to the tumor.

"We’re hopeful that administering IL 13-PE toxin will be the first and most significant step toward the reduction and eventually, the elimination of fatal brain tumors," said Dr. Piepmeier. "This technology is still in its infancy and we are still learning more about the long-term effects of how IL 13 treats these tumors and the prognosis for our patients. This is a big step toward giving hope to patients afflicted with brain tumors."

This research is sponsored by Neopharm, a biopharmaceutical company based in Lake Forest, IL, which develops drugs for the treatment of cancer and licensed IL 13-PE toxin from the National Institute of Health.
